After blanking, formatting, and recording to the CD-RW in cd0,
processes accessing cd0 hang.  For example, eject hangs in this
state, according to 'ps auxwwl':

>How-To-Repeat:
After initializing a CD-RW with mmcformat -B, mmcformat -O,
and recording a NetBSD release image with

        sudo dd if=a-11-Mar-2009-i386cd.iso of=/dev/rcd0d obs=2k conv=osync

subsequent commands that access the CD-RW drive, such as

        sudo eject cd0

hang.
