 I have a similar system (D945GTP).  Please try using an ACPI amd64
 install kernel; that should work.
 
 The problem is that, as far as I can tell, the mpbios code doesn't get
 along so well with this chipset and/or motherboard.  If you need an
 amd64 ACPI install image, let me know and I will build one.

 I have a hyper-threaded Pentium D.  The hyper-threading cannot be
 turned on or off in the BIOS, and the CPU does not appear to the
 system as 4 physical CPUs.
 
 BTW, there really should be ACPI install ISO images and/or floppies
 for amd64.  Many machines simply don't work without it.
