A kernel with debug options did not help.
I just built the kernel from release-3 as of about 8:00 AM MST June 24, 2005. This was cross built on a 3.0_BETA amd64 machine, but I have had the same problem building on a 2.0.2 i386 machine.

Reverting arch/mips/mipsX_subr.S to 1.15 lets it boot.
