Yes, the appearance of my card and its coupler matches your
description of a type 4.  I kind of suspected there were several
versions of the card, but never imagined there would be this many.

Your patch works fine with my card.  I hereby withdraw my own patch.

One suggestion: since the Linksys entry is now used for both Linksys
cards and some D-Link DE-650 cards, it would be less confusing if it
printed something like "ne0: Linksys EthernetCard or D-Link DE-650"
instead of just "ne0: Linksys EthernetCard Ethernet".
