No, it doesn't. Is it possible that your "pkgsrc" tree is not okay?
How many patch files are in "pkgsrc/graphics/tiff/paches"? There
should be three of them.

That's exactly what is done:

tron@lyssa:/usr/pkgsrc/graphics/tiff/patches>grep Makefile *
patch-aa:--- man/Makefile.in.orig       Tue Sep  2 19:54:41 1997
patch-aa:+++ man/Makefile.in    Fri Nov 28 02:19:36 1997
patch-aa: apps/Makefile:
patch-ac:--- libtiff/Makefile.in.orig   Tue Sep  2 19:54:19 1997
patch-ac:+++ libtiff/Makefile.in        Mon Dec  1 00:20:35 1997

As you can see all patches are applied to instances of "Makefile.in".
