It sure is netbsd-012394. I downloaded it from cray-ymp.acm.stuorg.vt.edu
from the new_kernels directory and I downloaded/decompressed it as binary.
That much I am sure.

2. 5 mb ram: 4-1mb simms, 80ns and 4-256k simms, 80ns
3. Quantum 85ELS and all hfs--80 mb drive
   Quantum Q250 with 32 mb root/usr, 4 mb swap and 4 mb hfs--41mb drive
4. MacPro Plus keyboard
5. Apple High Resolution video card--set at 1-bit; max 256 colors, 8-bit.
   Might be an upgraded 4-bit card....
6. 3COM EtherLink/NB; 10-base2 (thinnet) card; model 3C543
7. NEC 3FGx monitor
