Looks like this BIOS only sets up the default 1:1 irq<->ioapic mappings.
I made an error in sys/arch/x86/x86/intr.c that would make the fallback
case (not finding a PCI mapping, having to fall back to ISA) fail.

Can you update that file and try again? I think it's fixed now.
