You can't be running 1.4.x.. do you mean 1.4X? 1.4.x (i.e. 1.4.1, 1.4.2)
doesn't support this controller, but -current (of which 1.4X was one
incarnation) does.

Anyway, these kind of problems do not look like software problems to me.
I'm afraid your CD-ROM drive (or the CD in it) might be dying.
