I was having similar problems.  It turned out that the binary
on the install disk - the one that has everything and links
to each required program - wasn't being built correctly.  Do
a make clean in the distrib/i386/floppies directory and start
over again.  My binary was 32k on the install floppy which is,
of course, impossible.  Init failed to run so a reboot was
generated just likes the docs say it will.
