Subject: floppy problems

I keep having problems getting a file system onto a floppy :-(
Could someone give a failsafe and tested recipe for how to doit ?

What I've tried is:

disklabel and newfs onto /dev/fd0a (3 1/2" floppy), plenty of errors
like this

I've also tried using /dev/vnd0a, but no luck.

Is it at all possible to put a disklabel/filesystem onto a floppy at the 
moment ?
If it matters this is on an AHA1542B.

On the positive side I have no other problems running JUL01 -current.
The change to 4.4Lite has been very well planned and implemented.
