It doesn't for me, at least not with pkg_install:

$ /usr/pkg/sbin/pkg_info -V
20130902
$ sudo /usr/pkg/sbin/pkg_add -u pkg_install-20130902nb1.tgz 
pkg_add: package `pkg_install-20130902nb1' already recorded as installed
$ sudo /usr/pkg/sbin/pkg_add -u -f pkg_install-20130902nb1.tgz 
pkg_add: package `pkg_install-20130902nb1' already recorded as installed
$ sudo /usr/pkg/sbin/pkg_add -uf pkg_install-20130902nb1.tgz 
pkg_add: package `pkg_install-20130902nb1' already recorded as installed
$ file /usr/pkg/sbin/pkg_info
/usr/pkg/sbin/pkg_info: ELF 64-bit LSB executable, x86-64, version 1 (SYSV), dynamically linked (uses shared libs), for NetBSD 4.0, not stripped
