Here's what I've got:

a HP Apollo 400 series machine, I'm fairly sure the model is a 424e.  

The monitor label has A2094A on it, if that means anything specific.

There are two disks in it, both are C2235's (400MB or so).  One has HP/UX
9.03 on it, the other is fair game for getting this to work.

I'm trying to get these things to boot, using the makedisk utility with a
custom made disk label.  I think I computed things properly, but here's
the actual label file I'm using:

Thus, the ns is the number of 1k sectors per track, or 30.  I want 512byte
sectors, so it should be 60 sectors/track, right?

So I write this to the disk with makedisk and boot, choose the boot option
reflecting this disk and all I get is a blank screen.  Does this mean the
video card isn't supported?  Did I screw up royally somewhere?  Is there
any way to boot this thing off the network, instead?  I can't seem to find
any net boot option on the machine.

What more information can I find out to make helping me easier?  Like
finding out more information on what's actually in my machine?
